Person Centred Planning

Welcome to The Person Centred Planning in Sutton Website. Here you will find all sorts of resources to help you if you are the focus person of a plan, someone who is facilitating a plan or are simply interested in working in a person centred way.

According to the Valuing People Support Team, Person Centred Planning means "finding ways of listening to people to find out what is most important to them and what they want from their lives. The next step is to help people get those things". Many Local Authorities have someone responsible for helping them work in a person centred way. In the London Borough of Sutton we have a Person Centred Planning Co-ordinator. Although based in Adult Services, planning with adults with a learning disability, the PCP Co-ordinator also works with children with a learning disability who are in transition (14+) to help prepare them for what has historically been a turbulent period in people’s lives. Therefore, in Sutton, we are trying to focus on what our young people feel they want as they grow up.

"Person centred planning assumes that people with disabilities are ready to do whatever they want as long as they are adequately supported. The ‘readiness’ model is replaced with the ‘support’ model which acknowledges that everyone needs support and some people need more support than others".
