Housing opportunities for young people with SEN/disabilities
We hold 6-8 weekly multi-agency ‘workstreams’ to share information about (and improve the quality/variety of) pathways into housing options, including social housing, housing association accommodation, private renting, shared housing/ownership, shared lives and supported living.
Liaison with brokerage team and individual providers in finding voids in appropriate supported housing options.
Liaison with commissioning team in identifying gaps in supported housing provision and planning for the likely demand in such areas, and promoting uptake in LBS shared lives scheme.
Referrals to/liaison with housing department around ensuring appropriate support/information provision for people with disabilities in general needs provision.
Referrals to occupational therapy and speech and language services, travel trainers, and assistive technology teams to support the development of skills needed to live in supported, semi-supported or independent accommodation.
We have attended information sessions to promote up-take of Assistive Technology options.
Referrals to/liaison with partner agency SCILL in providing advice to young people and their families in managing their Personal Budget or recruiting a personal assistant to help people to continue living in their own own/with family.
