What should go into an EHC plan
EHC Plans will set good relevant outcomes, identify need/s and include provision to meet said need/s. They must be co-produced, describe positivity and what children and young people can do and tell the child or young persons story well and coherently. The EHC plan must meet the requirements of the Act, Regulations and the Code. An Annual Review will take place every 12 months (every six months for a child under 5) where parent/carers will be invited to find out about their child’or young persons progress and contribute their views. Any changes that are needed to the EHC Plan will be discussed at these review meetings, interim targets will be reviewed and new interim targets set.
Your child or young person's EHC Plan should be written by the multidisciplinary team working with your child or young person, and should include:
- You child or young person's medical condition, its triggers, signs, symptoms and treatments
- You child or young person's needs, including medication (dose, side effects and storage) and other treatments. It might include other considerations such as time and facilities needed to administer the medicines, equipment, testing, access to food and drink where this is used to manage their condition, dietary requirements and environmental issues e.g. crowded corridors, travel time between lessons
- Specific support for your child or young person's educational, social and emotional needs – for example, how absences will be managed, requirements for extra time to complete exams, use of rest periods or additional support in catching up with lessons, counselling sessions
- The level of support your child or young person might need in class
