This Dementia Action Week (19-25 May), Sutton Council, Age UK Sutton and partners across the health and care sector are launching a Dementia Support Hub which acts as a single point of access for support for people living with dementia, their families and carers.
The Dementia Support Hub has been developed by Sutton Council and Age UK Sutton following feedback from residents.
Residents can receive help from the Hub following a referral by a GP or by contacting the Hub on 020 4551 7337 or by emailing dementiahub@ageuksutton.org.uk.
Professionals from Age UK Sutton and the Council’s Admiral Nurse Service will be on hand to ensure that people living with dementia and their support network can easily access help and information. People who contact the hub will be supported by a Dementia Advisor who will help the resident access support and advice, acting as a guide throughout their dementia journey.
The Dementia Support Hub will form a key part of Sutton’s new Dementia Strategy which is currently being developed by the Council alongside local partners and community organisations, as well as those affected by dementia.
There will be further opportunities for residents in Sutton to shape the Dementia Strategy’s delivery and implementation plan over the summer.
Councillor Marian James, Chair of the People Committee, said:
““We know that one in three people with dementia do not have a diagnosis and it’s vital that we work with our community partners to improve this. Designing services that meet our residents' needs is crucial to this and I would like to thank the time and input of Sutton residents living with dementia, their families and carers for sharing their experiences that have helped in developing the hub.
We hope that it will make it easier for our residents to access professional support whether they be concerned about themselves or a loved one."
Peter Glass, CEO, Age UK Sutton, said:
“Age UK Sutton are dedicated to providing services to individuals diagnosed with early to mid-stage dementia, as well as their caregivers, who are navigating the challenges of maintaining connections and accessing vital services. The Dementia Hub is a cornerstone service of our work in Sutton.
Our focus extends to those at risk of isolation, early decline or those struggling to find the support they need. With the rise of dementia prevalence in Sutton, projected to increase by 39.8% by 2040, it is crucial that we provide the right interventions to help individuals live well with dementia.
As the community faces growing demand, we strive to create supportive environments that foster connection, alleviate caregiver stress and ensure everyone has the opportunity to thrive. Together, we can make a lasting difference.
We have been delighted to be integral in the development of the Dementia Hub in Sutton and look forward to seeing the impact it has for people affected by Dementia into the future."
